Pathways For Aging serves older adults (generally defined as ages 60 and above), family and informal caregivers, and community partner organizations throughout the Bay Area. Initiating contact may be accomplished by phone, email, or in-person visit to a partner site. There is no fee for our services and all interactions are held in confidence. Response is provided in English and Spanish; please be advised that language availability can run the gamut depending on the type of request and current staffing.
Who Are You?
Older adults (60+)
If you find yourself in need of assistance with transportation, benefits, housing, or related matters, our navigators will work with you to identify appropriate options. You may reach out by phone, in person at one of our partner sites (locations include but are not limited to senior centers and clinics in Oakland, San Francisco, San Jose, Berkeley, Hayward), or via email. Those who aren't up to speed on computers can still get the ball rolling—we'll take it from there.
Family and informal caregivers
We assist with navigating benefits-related documentation, ride scheduling, and housing referrals so that you can support your loved one without hitting a wall. Reach out by phone or email; we will clarify which programs your family member may qualify for and outline the application process, though actual eligibility is determined by the administering agency.
Community partners
For senior centers, faith communities, clinics, and housing providers: Pathways For Aging serves as a navigation partner for your older-adult clientele. We offer warm handoffs, co-located service hours (subject to prior arrangement and site capacity), and training to build your staff's capacity to connect clients to transportation, benefits, and housing resources—so you're not left holding the bag when clients need more than you can provide in-house.

We maintain regularly scheduled hours at various partner sites in San Francisco, Oakland, San Jose, and other Bay Area jurisdictions. To identify the site and schedule that best fits your situation, please contact us by phone. You may also inquire at your local senior center or your county's Area Agency on Aging—they can point you in the right direction.
