Pathways For Aging facilitates the empowerment of diverse older adult populations to achieve and maintain safe housing, financial security, and community connectedness through the provision of clear, compassionate navigation services pertaining to transportation, public benefits, and housing resources, thereby enabling enhanced mobility, access to entitled benefits, and aging with dignity and independence.
Pathways for Aging is a community-based 501(c)(3) nonprofit organization whose primary function is to assist older adults residing within the San Francisco Bay Area geographic region with the navigation of transportation, public benefits/entitlements, and housing systems. We position ourselves as a trusted single point of entry (or "front door") to facilitate the translation of complex regulatory requirements and administrative documentation into actionable next steps. All services are provided at no cost to the client and are conducted in a confidential manner.
Service delivery is conducted across all nine Bay Area counties—San Francisco, Alameda, Contra Costa, San Mateo, Santa Clara, Marin, Napa, Sonoma, and Solano—pursuant to partnerships with senior centers, faith-based organizations, health care providers, and local Area Agencies on Aging (AAA). Language capacity includes English and Spanish; please note that availability of in-language assistance may vary by program and is subject to staff capacity.
Our Programs
Transportation & Mobility
Individualized navigation assistance with respect to paratransit eligibility and enrollment, senior shuttle programs, volunteer driver networks, and reduced-fare transit options; ride-planning and scheduling support; and mobility coaching to reduce barriers to medical appointments and social engagement.
Comprehensive eligibility screening and application/renewal assistance for federal and state entitlement programs (e.g., SSI, Medicare, Medi-Cal, CalFresh, utility assistance). Periodic benefits check-ups are recommended upon the occurrence of qualifying life events.
Early-intervention support for individuals at risk of housing instability; referrals to legal aid, rental assistance programs, tenant-rights resources, and a continuum of housing options including aging-in-place supports through assisted living.
Older adults (60+) who are low-income, living alone, managing chronic conditions, or facing language or technology barriers
Seniors from immigrant, refugee, and historically marginalized communities
Family and informal caregivers who need clear answers and support
Community partners (senior centers, faith communities, clinics, housing providers) seeking a navigation partner for older clients
We Meet You Where You Are
Access is available via telephone, in-person at designated partner sites throughout the Bay Area (e.g., Oakland, San Francisco, San Jose and environs), or by electronic mail. Individuals who are not comfortable with technology need not be deterred—our staff will walk you through the process from soup to nuts.
